Why Rehabilitation Care is Best Done at Home 

There was a time when the only option for additional care after a hospital stay was to be transferred to a rehab or skilled nursing facility (or SNF). Contessa’s Rehabilitation Care at Home model changes that by offering a safe and effective alternative to the traditional skilled nursing facility stay. With this program, patients can return home and receive the same high-quality rehabilitation care they would receive in a facility. 

Our care teams are designed and equipped to restore patients’ health while coordinating all the services, doctors, and other specialists that they need for up to 60 days. Patients who qualify can enter the program with a referral from the hospital or their primary care physician. As part of their care, patients receive regular in-home therapy, nursing visits and telehealth consultations from physicians. 

For patients who were already receiving their hospital-level acute care in the home, our Rehabilitation Care at Home services allow a seamless transition from one level of care to another, without changing locations. Patients benefit from this easy access to an even broader spectrum of coordinated care. 

How does Rehabilitation Care at Home differ from Contessa’s other services?

Contessa offers a comprehensive care at home model that provides a full spectrum of care that patients would otherwise receive in a hospital or long-term care facility. We all want to exist within our lives on our own terms, in as healthy a way as is possible, which is what Contessa’s model allows. 

Just as Contessa’s Rehabilitation Care at Home model offers a safe and effective alternative to the traditional skilled nursing facility stay, our Recovery Care at Home model is designed to effectively replace a traditional hospital stay. While Recovery Care at Home provides hospital-level care to patients with acute needs in a care episode lasting up to 30 days, Rehabilitation Care at Home addresses patients’ rehabilitation needs with a care episode that extends up to 60 days.  

Recovery Care at Home and Rehabilitation Care at Home are part of our comprehensive care at home model. Contessa also offers Palliative Care at Home, a model designed for patients living with multiple chronic conditions and who would benefit from additional support. These services are targeted toward patients living with terminal, chronic or advanced-stage illness and aim to enhance their quality of life while receiving care for serious illness.

After building and scaling Recovery Care at Home, our Rehabilitation Care at Home program is a natural evolution in achieving our mission to make the healing experience more enjoyable for both those receiving care and those delivering it. It seamlessly fits into how we care for patients on a longitudinal basis rather than standard episodic at-home treatment.  

Transitioning from Recovery Care at Home to Rehabilitation Care at Home mirrors the often-organic progression that patients experience from one step in their journey to the next. It also bridges a gap to provide tailored care to patients who fall somewhere between our Recovery Care at Home and Palliative Care at Home services. In tandem with our other offerings, Rehabilitation Care at Home enables us to provide high-quality care across a coordinated continuum, ensuring patients achieve the best possible outcomes and receive the right care in the right place.
