How Rehabilition Care at Home Works
Contessa’s Rehabilitation Care at Home model offers a safe and effective alternative to the traditional skilled nursing facility stay.
Traditionally, many patients are transferred to a rehab facility after a hospital stay to receive skilled nursing level care.
With Contessa’s Rehabilitation Care at Home program, patients can return to their homes to receive the same, high quality rehabilitation care they would receive in a facility.
We build a care team that is equipped and ready to restore patients’ health while in the comfort of their own home.
Eligible patients can enter the program with a referral from the hospital or their primary care physician.
After opting into the program, patients are given all the technology and instructions they need to receive easy, convenient and safe care at home for up to 60 days. During this timeframe, patients typically receive regular in home nursing visits and telehealth consults from physicians.
