Employee Spotlight: Michele Mitchell, Admissions Partner

Meet Michele Mitchell, an Admissions Partner who works remotely in Contessa’s Virtual Care Unit. In her role, Michele collaborates closely with several of Contessa’s partner health systems — screening patients for eligibility and supporting Recovery Care Coordinators throughout the admissions process. 

With a background in social work and discharge planning across both hospital and home health settings, Michele brings a wealth of experience and a deep sense of empathy to her work. Her commitment to patient care and continuous growth truly stands out. 

We recently caught up with Michele to learn more about her journey at Contessa. She shared how purpose and impact shape her daily work and why she finds her role so meaningful. We’re excited to share her story and hope you find it as inspiring as we did! 

All About Me 

“I have a B.A. in psychology and am a certified case manager (CCM). I was a surgical/trauma social worker at an acute care hospital and a case manager at an inpatient rehabilitation hospital.  

Most recently, I worked as a clinical liaison for a home health agency, collaborating with social workers and case managers in an acute hospital. I loved being able to assist patients and families during what can be a very vulnerable time. Helping them feel supported and confident as they transitioned from the hospital to care at home was incredibly rewarding.” 

What I Love About Contessa 

For Michele, joining Contessa wasn’t just about the role – it was about the mission. “From my experience as a social worker, I know that many patients would rather not stay in the hospital,” she says. “I love that Contessa’s Recovery Care at Home program allows patients to thrive and recuperate in their own homes – providing better outcomes and fewer readmissions.”

A Culture of Growth and Support

Michele highlights the strong sense of collaboration across teams and markets. “Even though we’re remote, there’s a real sense of teamwork and comradery. It’s an environment where you’re encouraged to ask questions and continue building your skills.” 

She also embraces the chance to grow in her role. “I’m proud of how I’ve adapted as our markets change and expand. I really value how Contessa supports professional growth and encourages you to deepen your clinical expertise.” 

Life Outside of Work

When she’s not coordinating admissions across time zones, Michele makes the most of the outdoors: hiking, gardening and playing pickleball. “I can’t live without my family, flowers and my earbuds for music,” she says with a smile. 

Michele’s commitment to doing what’s right for patients, her trust in the team around her and her relentless desire to grow reflect what makes Contessa special — and what makes her a standout partner in our mission to bring care home.
